Shift available: Part-time from 6 p.m-6.30 a.m on Tuesdays and Fridays

Responsibilities

  • Observes and verifies the 6 rights of medication administration by making sure to have the right individual, medication, dose, time, route, and documentation on specified forms or records. (Determined by community policy and State regulations)
  • Provides medication administration services to the residents as determined by community policy and State regulations
  • Report and document a resident’s refusal to take prescribed medications
  • Maintains proper labeling, storage, and destruction of medications according to community policy and procedures
  • Obtains and records resident's vital signs (temperature, blood pressure, pulse and respiration rates), weight, as directed
  • Observes resident to detect response to specified types of medications and prepares report or notifies designated personnel of unexpected reactions
  • Answers resident’s call signals promptly to determine resident’s needs
  • Encourages residents to do things for themselves and to retain feelings of independence and self-esteem. Maintains dignity of resident in all instances
  • Provide resident services and assistance with activities of daily living utilizing a Person-Centered Care to assist in maintaining a positive physical and psycho-social environment for the residents
  • Perform or assist with ADLs, using care plans and flow sheets as a guide for bathing and personal hygiene, dressing and undressing, oral hygiene, nail care, shaving, incontinent care, toileting, changing of bed linens and positioning
  • Transfer residents to, or from, bed, chair, or commode. Transport residents, as needed, to activities of choice
  • Assist others with transfers and lifting, as required
  • Perform assigned restorative procedures. Provide range of motion. Assist with standing, ambulation. 
  • Regularly observe the residents physical & mental condition and report changes in resident to Director of Wellness and/or Director of Memory Care
  • Accurately document care provided on flow sheets and report all incidents and accidents
  • Perform emergency first-aid procedures when appropriate including basic first aid, CPR, Heimlich maneuver, etc.
  • Miscellaneous duties as assigned by Director of Wellness, Memory Care and/or Executive Director

Qualifications

  • Arizona Assisted Living Facility Caregiver Certification (it's not the same as a CNA certificate, it requires completing 102 hours of training before 2019 or 62 hours if it's recent and passing the exam)
  • Med Tech Certificate is preferred
  • Valid Arizona Fingerprint Clearance Card
  • Current First Aid and CPR certifications
  • Must be 18 years of age or older
  • Experience working with dementia or Alzheimer’s residents preferred
  • Must be able to work occasional nights, one weekend day and holidays as required and to be on call via telephone for nursing related emergencies
  • Excellent customer service skills
  • Sufficient fluency in spoken and/or written English
  • Pass post offer TB test
