Overview

Primary Purpose of Position:

 

To assist the Director of Wellness in achieving the goals and objectives for the senior living community by ensuring superior quality clinical services for our residents. To act as the primary subject matter expert in Dementia care programs and services.

Responsibilities

Duties and Responsibilities

 

  • Recruit, hire, train and manage a team of caregivers, medication technicians and activity assistants in Memory Care and Signature Services neighborhoods
  • Responsible for all staffing and scheduling of all caregivers, medication technicians and activity assistants in both Memory Care and Signature Services neighborhoods
  • Responsible to ensure the entire branded LivGenerations Memory Care program is optimized, which includes specialized team member training, dining services, life-enriching activities, and physical plant operations
  • Responsible for ensuring the environment is beautifully maintained always
  • Manage the day-to-day healthcare operations of the memory care program to ensure residents’ healthcare needs are proactively being met
  • Coordinate nursing/resident services to ensure the continuity of the resident’s total regimen of care
  • Effectively communicate with physicians and family members
  • Resolve concerns regarding resident care with all constituents in a timely manner
  • Provide hands-on resident care as needed or warranted
  • Perform ongoing nursing assessments/observations of residents’ physical and psycho-social needs, and coordinate with other departments to assure quality care
  • Provide continuity of care for those residents receiving home health care, hospice services, and other third-party healthcare-related services
  • Ensure proper and timely records reporting in your department in accordance with state and federal guidelines and company standards
  • Supervise and coordinate admissions, assessments, and care plans to assure accuracy, timeliness and completeness so they are personalized        
  • Perform on-site assessments of residents admitted to hospitals or skilled nursing facilities for treatment, and maintain contact with resident’s family and discharge planner with the view to returning to the community
  • Ensure compliance of resident confidentiality and HIPPA regulations
  • Document and report care provided and resident observations, including unusual or significant changes in physical, mental or behavioral conditions
  • Maintain a working knowledge of state regulations and policies and procedures, dictated for compliance
  • Assist with medication and treatment administration, and ensure medications are administered per physician orders
  • As directed, review and coordinate completion of incident reports that are health and injury related, and develop/monitor plans of correction in accordance with policies and procedures

Qualifications

Job Qualifications:

  • Minimum of three years of progressive Nurse Management experience in Assisted Living including experience working with dementia or Alzheimer’s residents
  • Demonstrated working knowledge and experience with Geriatric Nursing and care services for a wide variety of residents
  • Strong interpersonal skills with the ability to communicate effectively to a wide range of audiences, both internal (care staff, nurses, executives, etc.) and external (families, residents, physicians, vendors, etc.)
  • Must be able to work occasional nights/weekends and holidays as required and to be on call via telephone for nursing related emergencies
  • A valid Arizona caregiver certificate or LPN license, in good standing preferred
  • Current Fingerprint Card
  • Pass post offer TB test
  • Current First Aid and CPR certifications
  • Must be able to obtain a food handlers card within 30 days of employment
