Pay: $25.00 to $30.00/HR

LivGenerations is the premier luxury senior lviing community helping seniors in the valley live fuller lives!  We are seeking a compassionate Weekend Wellness Nurse to oversee the community, team members, & residents over the weekend. You will be providing hands on care to our residents, overseeing our care team members, and communicating with physicians/family members of our residents.

  • Manage the day-to-day healthcare operations of this IL/AL/memory care community to ensure residents’ healthcare needs are met
  • Coordinate nursing/resident services to ensure the continuity of the resident’s total regimen of care
  • Effectively communicate with physicians and family members
  • Resolve concerns regarding resident care with all constituents in a timely manner
  • Provide hands-on resident care as needed or warranted
  • Perform ongoing nursing assessments/observations of residents’ physical and psycho-social needs, and coordinate with other departments to assure quality care
  • Provide continuity of care for those residents receiving home health care, hospice services, and other third-party, healthcare-related services
  • Ensure proper and timely records reporting in your department in accordance with state and federal guidelines and company standards
  • Supervise and coordinate admissions, assessments, and care plans to assure accuracy, timeliness and completeness so they are personalized                  
  • Perform on-site assessments of residents admitted to hospitals or skilled nursing facilities for treatment, and maintain contact with resident’s family and discharge planner with the view to returning to the community
  • Ensure resident confidentiality and HIPPA regulations are compliant
  • Document and report care provided and resident observations including unusual or significant changes in physical, mental or behavioral conditions
  • Maintain a working knowledge of state regulations and policies and procedures, dictated for compliance
  • Assist with medication and treatment administration, and ensure medications are administered per physician orders
  • As directed, review and coordinate completion of incident reports that are health and injury related, and develop/monitor plans of correction in accordance with policies and procedures



  • Active LPN licensure in the State of Arizona, in good standing
  • Minimum of three years of progressive Nurse Management experience in Assisted Living, including experience working with dementia or Alzheimer’s residents
  • Demonstrated working knowledge and experience with Geriatric Nursing and care services for a wide variety of residents
  • Strong interpersonal skills with the ability to communicate effectively to a wide range of audiences, both internal (care staff, nurses, executives, etc.) and external (families, residents, physicians, vendors, etc.)
  • Must be able to work occasional nights/weekends and holidays as required and to be on call via telephone for nursing-related emergencies
  • Current Fingerprint Card
  • Pass post offer TB test
  • Current, valid driver license, with clean driving record
  • Current First Aid and CPR certifications
